I still use Mono on my Model 1/Model 1 Sega CD/32x setup because I don't have the mixer cable.

Headphone out... simple and works well.
Was less than $10 at Radio Shack for the cable.
Yet another biased poll. ;). I use a Model 2 (with a model 2 Sega CD and through a 32X), but with stereo AV cables.
on my HD Model 1 and Jap Model 1 I use the headphone Jack, on my X'eye I use the RCA Jacks.
I always play my Genesis games with headphones
For Sega CD games I use the Model 1's headphone jack
For Genesis/SMS games I use the mixing cable and use some rca female to 3.5mm female thing for ultra clear audio.
Everyone should have a Sega CD. That's how I have Stereo sound set up with my Model 1. Not to mention I have the 32-Bit adapter to go with it to provide the ultimate '90s multimedia experience! All 5 games that support it! :p
I use a mixing cable to my Mega-CD, and from there I have two RCA jacks into a separate pair of speakers and a bass booster.
I bought an Aux cable and attached a 1/4th inch adapter to it. And hooked my Genesis into a Bass Amp.
RGB SCART cable with integrated headphone plug. Best video and audio quality in one go.
RCA out jacks come standard on the X'Eye. So that's what I use.
I use a model 2 PAL Megadrive with region swith and RGB cable. The most sexy console ever made :)
Mine has been modded for the RCA jacks.
Why mod for RCA jacks when you can just use a splitter cable from the headphone jack?
If I'm using an actual Genesis I'll use the Sega Cd's audio outs. Most of the time I use an X'eye with a SCART cable.
CDX line out with a 3.5mm to RCA cable.
I use the headphone jack for stereo audio from my model 1, and I use the RCA jacks from my CCAM-modded model 2.
I use the headphone jack, which is connected to a 70s Kenwood Amp that drives a nice
pair of big shelf speakers =)
Stereo amps with wooden decorations on the sides and massive metal plates on the front
make your music sound better!

I have a special A/V cable that has has a addition headphone plug that plugs into the headphone jack and feeds the sound into the Right/left audio outs. Got it years ago before the model 2 existed. That said I would guess you could get similar results from something like this:

Computer speakers can work well too, but the good ones would need it's own power
I use the headphone jack with the sega CD. (but don't bother with the mixing cable as there's little point unless you like the added analog filtering)
You also left off one option:
Audio from a Model 2 Sega CD with a model 2 Genesis. (it mixes internally and has full quality audio for the CD portion and improves the model 2's sound slightly too -varies by model too)
I have a model 1. I know I'm capable of getting "stereo" from the headphone jack but I just don't mind the mono. Although I have to say I love the stereo for SNES.
The best experiences I've had is with a decent pair of headphones. (both sound quality in general and stereo effects)
I Also use my model 2 with a 32x and i use a mixing cabe to get the sound from the mega cd, and beter i have it connected to a great sorrund system. Playing mega drive on 2.1 sorrund sound never felt so good. XD
I have a model 2 genesis and Sega CD 2. If I use stereo audio cables with the RCA out on the back of the CD, will that make the CD's and Genesis' sound true stereo? Does it make a big difference over mono?
You can get stereo from the model 2 AV port too, but yes, genesis sounds are in stereo too. (mono AV and RF are the only ways you'd end up with mono output from the console)
A lot of games use stereo effects in music and sometimes sfx.
The added benefit fromt eh Sega CD RCA jacks on the mode 2 is that the genesis sound gets passed internally and mixed/amped in the sega CD, s there's a small improvement over the stock genesis (some noise and distortion may improve on bad model 2s -maybe also on the better sounding late models), but more importantly, the CD's sound (from CD audio and onboard PCM sound chip) will be very clear and crisp.
On a model 1 it makes no difference, sound is always mixed from the genesis and using he mixing cable passes it through. (so not much different than using the headphone jack)
However, using the RCA jacks without the mixing cable will give the CD sound only, but at superior quality in some cases (VA7 model 1 especially with bad sound, but also due to the rather poor headphone amp adding a small hiss on all model 1s).
So you get clean CD audio and PCM: probably most useful for listening to Audio CDs. (possibly some games that only use the CD audio)
I just use the headphone jack on my non-HD Model 1. Hopefully, in the future, I can hook it up to one of these:

Both my Model 1 and Model 2 have been modded for RCA stereo output.
Really? I thought that even with the stereo composite cables, you only get dual mono, with both speakers outputing the same stuff, as opposed to real stereo where the speakers output separate parts of the music independently. That's how Mark from CGR explained it, anyway.
