Anyway, in all seriousness, this isn't TF3 where you can beat it on your first play through, you have to practice. Of course, the better you get, the harder the game will get (an interesting thing called "rank," where the longer you survive without dying, the more the game increases the difficulty a bit to try and kill you). Yes, this game's a challenge, but not an unfair one. You simply need to get better by playing it more, learning where the enemies come onto the screen, the boss patterns, and so forth... just like any other shmup from that era. Only this game does it on a bigger scale.
So just relax, sit back and enjoy the Genesis at its shmuping best. And when you're ready, crank the difficulty up to "Mania."
I also remember a couple of cheats, like 99 lives and all weapons, but since I didn't use them for more than 15 years now I don't remember what's required to enable them.
@ TheCoop: Personally I don't think Thunderforce III is much different. If you never played a shmup before you definitely won't beat it on your first try. It may not require anywhere near as much memorization than Thunderforce IV but it still won't be a walk in the park.

most games in the 16-bit era require shitloads of practice, patience and basically dedication to get your "rewards"...to me - thats the inherent nature of this whole era of gaming!
TF4 IS a dope game.....and mannn....it IS fun......
but yeah, ur right....its not easy to begin with, thats for sure...but stick with it yo!!
i can think of loads of games that i played that were so fuckin hard that i literally nearly of a heart attack through frustration....some of them became my favourties because of that very reason - persistence is key (as it is in most aspects of life, no?) :)
...in fact, this reminds me of the time i broke my big toe because i kicked my bed after loosing my temper when playing the ridiculously hard - Wardner...(i did beat it eventually though, and grew to love that too! although playing it these days reminds me why i did in fact break a bone, and could easily see it happening again to be honest)
if you think TF4 is difficult - try that motha-uffer!!!!....hardness guaranteed! *phew*

Set "stock ship" to zero for 99 lives. Pause the game while playing, and press up, right, A, down, right, A, C, left, up, B, up to get all the weapons. Then press right to get the C.R.A.W.s.
I'm hardly a shmuping god (not even remotely close really), and back in 1990, I was still pretty new to the genre. Even so, I was able to beat TF3 on Normal without continuing on my first try. It's just not that hard of a shmup. It's a great game, but not difficult. In fact, it gets recommended a lot to beginners over on shmups.com because of this.

That is a very nice list. Games like Comix Zone or Jurassic Park won't be on everyone's Top-22 list of course, but overall that is one great package of Mega Drive goodness right there.
However...
You're still missing Golden Axe, Streets of Rage and Revenge of Shinobi!
May I recommend the Mega Games 2 compilation with those games all on 1 cartridge ? It even does feature the original artworks of all three games on its label!
There's also the 6-PAK cartridge which got those three games plus Sonic 1, Super Hang-On and Columns. And it does also feature the artwork of all six games on its label.
EDIT Streets of Rage and Revenge of Shinobi should be reason enough, but here's a reminder of the epic epicness of that ol' 1989 Golden Axe port:
