Dazzled: Sonic 2. I could not believe how fast that game was when I first played it. It completely changed my view of platformers(I played the Mario games a lot before I got Sonic 2, and since then, they've just been sitting around collecting dust).
Surprised: Nothing to say for this
Scared: Playing any Metal Gear game. I'm always afraid I'll be spotted by the enemy.

Not just the graphics, but the general style and gameplay had a significant attraction about it.
The fact that the only console we had at the time was an NES eccentuated that a bit as well, but it's not like I'd been sheltered from 3D (we had a fairly decent PC from 1993 onward) but that likely played a role nonetheless. (we didn't get our SNES until the end of '96 iirc and no N64 for another 3 years aside from renting)
I know I've been surprised or scared, but I can't remember more specific examples. (I know Silent Hill 2 and 3 had moments that apply, but there's some other stuff that fits even more, especially from when I was younger, but I can't put my finger on the games ATM)
