Depends. If I had a dedicated CRT TV to use with my MD than I would keep it. If I had to constantly hook up the MD to a shared TV I'd say fuggit and go with the Nomad.

Because they are better than PSPs. It's true. The only thing Sega did that was really stupid was not to make them run off 12 AA batteries in order to run Sega CD games for a few minutes; they could have made a special mark 3 version to attach to the back of the Nomad. I could just about live without 32X games, but the idea that I could play Sonic CD for 1/2 an hour tops on a bus in the 1990s is just too appealing to me! They committed marketing suicide by not doing that one.
I love my Nomad but bye-bye!
I'd keep the Nomad, cause then I could play it on the TV and take it with me. I guess it helps that I have a beautiful new screen in my Nomad that doesn't suck the batteries dry.
There are many more variables than just which one do you keep. If you need to play 32X, obviously you keep the MD. If you don't have a 32X or games, you may want to keep the Nomad. I see people complaining about the batteries on the Nomad - so why don't you get rechargeable batteries? Between rechargeable batteries and the AC adapter, I spend zilch on batteries. That last time I used regular batteries in my Nomad was when I tested which uses less power - the MD-Pro or the Everdrive-MD.

If by "Mega Drive" you mean a 50Hz PAL Mega Drive then I'll take the Nomad, no doubt about that. If a Mega Drive is the Japanese version, a modded PAL MD or a Genesis, I'd agree with the rest of you and go with that. - I will never ever play in 50Hz again!
