Looks like Shit

I saw a cart-only Sonic 3 one time that was listed as "very good" despite some kid putting a sticker of a cricket playing a musical instrument on the label. So yeah, this doesn't surprise me much.
I really hate how liberal US sellers are with their descriptions. I got a supposed "very good" copy of Mega Turrican for Christmas that at best would only pass for "good" after I cleaned all the crap off of it.

Those stains look almost like they may be permanent (shoe shine?). If so, and the game plays fine, then it is at most in "fair condition". But maybe this is what the seller considers "very good". It is certainly within the power of every viewer of that auction as whether to bid or not. "Very good" to one person may not be the same to another. Just trying to extend the poster of that auction some grace...
