Streets of Rage 1 (look for the 6-pak cartridge)
Golden Axe (6-pak again)
Golden Axe 2
Golden Axe 3 (Japanese import only, but definitely worth the trouble!)
Bare Knuckle 3 (original import version of Streets of Rage 3 which was heavily altered = completely ruined for the Western release, again well worth the trouble)
What do you mean by 6-pak cartridge? could you clarify please?
Streets of Rage 3 isn't that deplorable but not worth the usual asking price. I don't think Bare Knuckle 3 was very good to begin with. The first two are all you need.

Separation Anxiety
Some sleepers:
Alien Storm - The original arcade game was designed by the creator of the original Golden Axe arcade game (Golden Axe 2+3 were made by a different designer/team). The Genesis port of this game is nothing spectacular but overall still rather solid. Well worth tracking down a copy if you're looking for another game similar to Golden Axe 1 and 2.
Blades of Vengeance - Similar to Altered Beast in the sense that you're moving on a 2D plane (unlike in a 3D space like in Golden Axe / Streets of Rage). Great graphics and sound but personally I think it's more fun when playing alone.
Captain America and the Avengers - Arcade port. Rather primitive by today's standards.
Growl (aka Runark) - Another arcade port. And again rather primitive by today's standards. 1P only.
Power Rangers: The Movie - One of the very few Mega Drive / Genesis games developed in Japan but released only in the US.
Sailor Moon - Import only.
Shui Hu Feng Yun Zhuan - A Chinese original. Great graphics and sound, solid game play.
Two Crude Dudes (aka Crude Busters) - Arcade port. Again similar to Altered Beast. Fantastic soundtrack!
Mortal Kombat on the MD is awfull, but so awful it is worth looking at... The SNES did it so much better but I will have to agree with other posters on here. The Streets of Rage (AKA BARE KNUCKLE) games are AMAZING for the MD

GET ALL OF THESE:
- The streets of rage series, but get the japanese version of the 3rd (they messed up the western releases)
- Comix Zone
- Golden Axe 1 OR 2 (they're pretty much the same), get 3 if you don't mind boring characters and mounts, horrible backgrounds, and shamefully disappointing spell effects. It has many more moves available then the previous 2, so take that into account.
- TMNT: The Hyperstone Heist - not as good as Turtles in Time, but the music is some of the best of the 16 bit generation, and the game is still very fun.
- Final Fight (Sega CD)
- Splatterhouse 3 is one of the coolest games I've ever had the pleasure to play, even if the timer gets annoying (at least you don't lose the game for running out of time, just worse endings)
- Shui Hu Feng Yun Zhuan - pretty good for a chinese pirate
MEDIOCRE:
- Battletoads & Double Dragon

I was wondering the same thing. Battletoads seems to be a pixel perfect port of the NES original, much like Capcom's The Wily Wars, or Nintendo's Mario All-Stars.
What you definitely should avoid would be Double Dragon 1 + 2 + 3.
Yeah, the DD games on genesis are all equally awful.

The Punisher! damn this game is a gem should be on everyone's list yet it is on no one's.
SOR 1 <- Great but dated graphically
SOR 2 <- Near Perfection
SOR3/BK3 <- the more you play the game the less you like it
Alien Storm <- I like this more then the Golden Axe games
Captain America <- Slippery controls but fun
Power Rangers Movie <- Solid gameplay, but extremely easy and uninspired
Splatterhouse 2/3 <- 3 more so then 2
Shui Hu Feng Yun Zhuan <- Knights of the Round on the Genesis W/Blood
BattleToads and DDragon <- Better then the Snes version
The Punisher needs it's own message
Get the game. It will set you back a penny, but for console console brawlers there is nothing close to this in terms of violence and mayhem. The game play never gets dull because of Bats, Pipes, Knives, Pistols, Machine Guns, Grenades, Swords, Axe's and Flame Throwers. Not to mention a good variety of regular moves and special moves.
The Punisher is undeniably average.
The difference is probably the fact you like the music which is rare. Once you have heard the crap music enough it detracts so much from the game. So does fighting in boring dull backdrops for 90% of the game.
Streets of Rage 2 the backdrops were so interesting and neat "Presentation is so key in a brawler" and from the Character Select screen on BK3 is sub par.
These are actual beat'em ups. Some people are listing fighters and action side-scrollers.

Here's a few suggestions for lesser-known Genesis beat 'em ups:
DJ Boy isn't the greatest game by any means, but it has a quirky charm about it. How are you going to go wrong with a beat 'em up played entirely on roller skates?!?
Hokuto no Ken (aka Fist Of The North Star) is one of the bloodiest games in 16-bit history. Sega took out all of the blood (and most of the fun) for the Western release of the game, called Last Battle.
Mystical Fighter is probably the only kabuki-themed beat 'em up in existence. The gameplay is nothing too special, but it certainly scores style points.
The Yang Warrior Family is a decent albeit difficult game. It could've been really good if programmers were actually given ample time and resources to polish it. As it stands, I enjoy the feudal China vibe - and it's one of the better unlicensed games for Genesis.
The Master System has a few gems as well. All of these games can be played on the Genesis with the Power Base Converter:
Black Belt is the Westernized 8-bit version of Fist Of The North Star. You fight the bosses in a versus style fighting game, which is quite impressive given that the game came out in 1986.
Renegade is vastly superior to the NES version.
Vigilante is a decent arcade port.
Kung Fu Kid is the happiest kung fu beat 'em up I've ever played.
Dynamite Dux stars a gun-toting duck and loads of bizarre foes. The Punisher can't hang with that.
Can't go wrong with any of the SoR series, although everyone has their preference. I like them all.
TMNT Hyperstone Heist is fun, but a little pricey for what amounts to a decent brawler.
Comix Zone is top notch, and I believe fairly inexpensive. It is a very creative and fun brawler, and one of my favorite Genesis games.
It was released in Europe as well.
I wholeheartedly disagree. I've finished this game more times than I can count, and I still do it again nowadays, like once a month or so :P
I don't care about its music (which is clearly inferior to BK2) but the gameplay is so solid and varied you can't go wrong with it.
I prefer these:
- Streets of rage 1, 2, 3
- Spiderman vs Venom Maximun Carnage
- Turtles Hyperstone Heist
- Double Dragon 2
Many suggestested games are more like platformers and not beat em ups. These are in my mind the best beat em ups for Mega Drive.
How ever here are my choice of platfromers:
- Adventuries of Batman and Robin
- Alien Soldier
- Probotector
- Universal Soldier
These are the ones I play these days a lot.
Bare Knuckle 3 and Golden Axe 2 are my favorite MD beat'em ups ever.
My favorites are Turtles Hyperstone Heist and Golden Axe II.

Altered Beast is a true classic, and has its unique bizarre "atmosphere"... I'd say that it's a very unique game. Many people have fond memories about this...
Two Crude Dudes (Crude Buster) is a top-quality Beat'em Up! One of the finest Data East's gems... The Sakimoto's sound driver is another plus of this game. BUT maybe you don't know how to appreciate the 80's feeling of a hard-ass arcade-style game like this...
Altered Beast is the definition of average.
The only cool thing about Two Crude Dudes is the sakimoto music, besides that it's 100% average. Still better than all the other games on the list though.
Here's my favorite beat 'em ups:
Streets of Rage 1 - This is an awesome, awesome beat 'em up. Don't let anyone persuade you otherwise. It protrays urban decay and a gritty atmosphere like nothing else. The more I play it, the more I like its look and feel. The soundtrack is the best in the series too.
Streets of Rage 2 - Even better than 1. In fact, it's the best beat em up I've ever played and that comes from someone who owns the Violent Storm and Punisher arcade pcbs.
Streets of Rage 3/Bare Knuckle 3 (preferred version) - Different from the others in look and feel but still uniquely awesome.
Two Crude Dudes - It's a heavily watered-down version of Crude Busters but it's really cool and fun to play on Genesis. What makes it unique is the huge number of objects you can interact with in the various stages. The is also definitely not a button-masher. Timing your throws and hits is the key to success here.
The Punisher - Again, very watered down from the arcade original but still really cool with a ton of different weapons and moves at your disposal.
Please guys, no acronyms! :daze:

I was just about to post how no one seems to separate Beat 'em Up and Hack 'n' Slash as two separate genres. To me, they are very different.
Beat 'em Ups are games in which your character has to use his her fists/feet/etc. to take on swarms of enemies. These are typically side-scrolling and often feature two-player co-op. The character may or may not be able to pick up weapons from destroyed items or fallen enemies. Oh, and there is always an elevator level ;)
Hack 'n' Slash games are often swords 'n' sorcery games in which your character is equipped with a weapon (sword, axe, mace, etc.) that he or she never loses. Levels are often reminiscent of Beat 'em Ups, but they typically don't have the same diversity in their move sets. And there are much fewer elevator levels :)
By the way, Streets of Rage 2 is pretty much the end-all, be-all of Beat 'em Ups for me. I never tire of playing that game.

Golden Axe and other "hack 'n slash games":

- the character may or may not be able to pick up weapons/powerups from destroyed items or fallen enemies - check
- there is always an elevator level - from a gameplay mechanics point of view, why does this matter?
Hack 'n slash games are a totally different genre (think Diablo, God of War, Titan Quest, etc.), which isn't present on the Megadrive. The closest thing to a hack 'n slash game is The Immortal.
