Manhunt 2 on consoles was rated M. They had to reduce it to an M rating to publish on consoles; I had forgotten about those very, very few games that got AO for violence, but yeah, there are those two and almost no others. It's a rating mostly for games with sex in them, because this is America and violence is fine but not sexuality.
On that note, that Hatred is allowed on Steam but not any AO or would-be-AO-if-they-had-a-rating porn games or adult Japanese visual novels (only the all-ages versions are allowed, not the adult versions), really shows how Steam sticks with that aforementioned American 'violence is okay, but not sex' thing.
