I've always found the boss statues in Super Metroid very creepy. Who built that room anyways? I don't think the Chozo would worship Ridley or the others, quite the opposite, Ridley attacked the Chozo planet and killed Samus parents on her planet too.

The alternate world in the first Silent Hill is still one of the creepiest places I have ever seen in a video game.

I've found many a setting on World of Illusion to be extra-creepy with a side of chips.
[edit] You see, if the game is supposed to be creepy in the first place, or at least cries out "esoteric", then I wouldn't be as creeped-out as I would with a Disney cartoon game.
The alien level in Streets Of Rage 2 creeps me out. All that fog and wild g-funk really set the tone. Plus you can hear that giant alien "purring" from a distance. Creepy!
I just played Streets of Rage 2 for the first time the other day. That's quite a turn in the canon.

I can't find video of it, but this stage in Phelios for the Genesis always kind of creeped me out. Maybe it's the music, I dunno. Kind of has that odd Southern Oracle sound to it in the Neverending Story movie. Speaking of Phelios, if you have never played it and like vertical shooters I highly recommend it. I always found the factory at the beginning of Snatcher rather creepy.

creepy shit, that place just oozes evil in a way.
The cut scenes in-between levels in Altered Beast are totally creepy.
Dragon's Fury is as about as creepy as pinball gets. Gorgar is as creepy as pinball gets.
I always feel kind of weird about playing Devilish. It'd have been interesting to be a fly on the wall for the brainstorming and marketing of that game. "Let's make a demonic game of Breakout!"
