I'd probably have to say the music from Valestine Castle in Ys III for the Genesis is one of the best I've heard.
I can't really pick one Mega Drive game that has the best soundtrack, not even a top three.
As far other games are concerned, all installments of Baldurs Gate have absolutely awesome
music, with the second game and it's add-on being the best probably. If PC games are allowed,
I'd say Baldurs Gate 2 together with Throne of Bhaal has the best soundtrack.
The password theme in MTPO when Little Mac is jogging and Doc Brown is on the bike. I always pictured some little Japanese guy hunched over a Casio keyboard and just going nuts. The song really holds up to this day, maybe it's just me though.

My favorite soundtrack is Final Fantasy 6, it's just so great. Sometimes I replay the game just for the music.
Curryman has some more of the games I love above, although I like Dracula X PCE's soundtrack better than SOTN's. I consider it to have the definitive Castlevania Soundtrack.
I'm very biased towards Sonic, so I have to exclude his games for a fair answer: but Mega Man X is one of my favorites. Chrono Trigger too... I was very anti-Nintendo during my childhood, but I believe the SNES had some of the best videogame music ever (F-Zero anyone?)
Specific tracks would be Revenge of Shinobi's The Shinobi and Make Me Dance.
Entire soundtracks would include All Shinobi games, Crackdown, and now I'm tired of searching.
Did they ever release an arranged version of the Revenge of Shinobi soundtrack? I love the composition, but it sometimes sounds like you're standing in a hallway. I assume it's due to Koshiro not being as familiar with the hardware as he was when he made SOR 2 or something like that. ActRaiser is like that too, similar styles of music but part 2 has better audio (not necessarily better music.)
I don't hear any echo effects in the Revenge of Shinobi soundtrack over my Genesis 1. Act Raiser sounds as sharp and non-filtered as any SNES music. I am not aware of any arranged soundtracks for Revenge of Shinobi outside of OCR stuff.

EDIT: Also Sheath, The Shinobi as featured on The Super Shinobi and Works. All the RoS tracks on that album are the PC-8801 source versions.
It's not really an echo, it's just that it's not as crisp as his later stuff. I'm playing the youtube clip above and I hear it. Those arranged soundtracks Cornholio posted just sound weird to me though, I'll take the originals.
As far as ActRaiser, compare the openings for the two games, since the themes are similar:
I'm not saying the first game sounds bad, it's quite awesome, but the second one sounds better on a technical level. I actually like the first game's arrangement better.
