Use a Game Genie as a pass through and install a switch.
The region switch mod is all over the internet. Very simple to preform.
cut the cart slot a little bit and most games fire right up on a US genesis. Installing a region switch is easy to do too. Or yeah just like lastcallhall said, use a game genie and that'll work for all the region locked games with a code in the game genie.
Makes me wonder. Has anyone found game genie codes to make games play in Japanese? Even for games without a region lock and/or shape issue, it'd still be useful for those who can't mod.
Buy a japanese megadrive.
my preferred method, providing you've a non-TMSS HDG MOD-1;
open the body up, slip the twin-lead connector for the POWER LED off, to free the top-body half from the rest of the console,
unscrew and remove the cart-slot-door retaining frame and door halves from the upper body, and carefully prize away at the slot gap
with a good X-ACTO until it's sides are rounded off enough to accept JPN MD carts, like thus:

Megadrive games fit into a 32X without modification. It's awesome.

that contradicts mine. megadrive games fit the 32x i'm using too.
We may be headed toward a new discovery here. Inconsistent results of this nature always have an answer.
The fact some US 32x units accept normal japanese games but others apparently don't (I don't own the latter 32x type to check) doesn't feel like coincidence.
After all we're talking about standardized physical dimensions for rigid objects produced on the scale of hundreds of thousands.

Japanese carts don't fit in my 32X either. I can get them into my Nomad with a little bit of jiggery-pokery, but it feels like it could damage one or the other.
For those whose JP carts fit in their unmodified 32X, is the model number of your unit MK-84000A. And, for those whose don't fit, is your unit model MK-84000. I have both. In the MK-84000A model the JP carts were barely able to fit, but the opening of the MK-84000 model is about 1/16 of an inch smaller and the JP carts won't fit without breaking the unit.
