Honorable mentions go to The Adventures of Batman & Robin, and Contra: Hard Corps, as they tie for fourth.
Ranger-X isn't exactly a run and gun. The first stage is run and gun then after that its more hide and seek than anything else.
Alien Soldier
Mega Turrican
Those are the only two I've played very much. But so far, Alien Soldier is my favorite Genesis game of all time.

^Most will agree those are the top 3 Run 'N Gun games in terms of quality. It doesn't get any closer to 10/10 than those three.

I really could've just put three Mega Turricans up there, though. I love that game. I'm sure I've played it at least 10 times more than Gunstar Heroes and 20 times more than Ranger-X.
Contra Hard Corps: so much mindless violence, I love it.
Gunstar Heroes: I do not own this yet, but soon it will be pursued.
Midnight Resistance/ Mega Turrican: yeah I tie these for third, so sue me.
