This week I bought a "High Definition Graphics" Model 1 Genesis with S-Video and stereo jack modifications. It also has a overclock switch that gives the option to set to factory 7.6 Mhz or an overclock speed of 10 Mhz. There is a Japanese/English region switch and another 50/60 Hz switch that allows for fluid PAL gameplay. The cartridge slot has been seemlessly widened to accomodate Japanese carts. Overall I feel I'm set to enjoy my collection in a whole new light. :D

I modded my early model 1 by removing the YM2612 :D Does that count?
My HDG model 1 Genesis is exactly as it was when I first took it out of the box in 1989. Everything still works, and I keep it clean :D
I've modded a few of my Genesis systems:
-A pre-TMSS High Definition Graphics Model 1 with a widened cartridge slot and Stereo A/V jacks(board revision VA3 - will soon make a Japanese MegaDrive lookalike out of it)
-A non-High Definition Graphics Model 1 with region switches, S-Video, widened cartridge slot and a green power LED(board revision VA6)
-A Genesis Model 2 with TmEE's Audio Mod circuit(board revision VA1)
-Another Genesis Model 2 with S-Video(board revision VA2)
-A third Genesis Model 2 with S-Video, a blue power LED, widened cartridge slot, US/JAP region switch, new cartridge slot, and MANY replaced/jumped parts(board revision VA4)
-A Genesis 3(board revision VA2) with the GN Twin clone's Genesis board replacing a broken Majesco motherboard
I'm also planning on making a Japanese MegaDrive 2 lookalike out of the numerous Genesis 2s I have.
I own a Genesis 2 + 32X combo with region and Hz switches, and which uses only one power supply. It's pretty handy.

nope but here shortly after a couple games get finished I am selling my current setup and next time around will likely go for a modded setup I want svideo and an overclock!
Region switch, PAL/NTSC switch (why not?) and a doctored cart slot to play Japanese games.
That is all you really need in life.

I've got a region-modded Mega Drive.
I have a region modded MD1 and MCD2. Not perfect (it's a bit huge!) but very useful for Mega CD imports - the CDX cartridge is hit and miss.
I used to run region locked 32x games by plugging it into the top of a Mega Key, which worked but always felt like danger.
