I am trying to find out what the rarest Sega Genesis games ever made? Everytime I google it in the search box I get alot of different answers. Is there some type of rarity guide that accurate? I enjoy collecting genesis games.

I believe digitpress has rarity guide but their site is down at the moment
Pier Solar has got to be near the top of any rarity list. There are only ca. 2500 copies
ever made (of the first run) and many of those aren't going to be for sale anytime
soon, because dedicated collectors have bought them. Look up ebay and you'll see
how extremely overpriced those things are atm and how many copies are for sale in
total, it's not going to be more than 10 I assume.
Also any game that's Japanese only and has no western release what so ever ought to
be very rare.
The accuracy of rarity guides is questionable, because rarity has a tendency to change, but here are a few games that are safe bets for the "rare" to fairly uncommon list (not counting imports):
MUSHA
Michael Jackson's Moonwalker
Pier Solar (first printing)
Beggar Prince (any printing)
Splatterhouse 2
Splatterhouse 3
Punisher
Contra Hard Corps
Castlevania Bloodlines
The bottom 3 aren't really super rare, but will cost you around $50 at least if you want them CIB.

Shinobi - the name means stealth.
A little off track but rarity is hard to access for example for a couple years I remember reading Final Fight Tough was way harder to find then Final Fight 3 on ebay the last couple months some hoarders must of started to sell their copies because there always seem to be a couple CIB auctions. Final Fight 3 CIB hasn't had an auction in months only BIN gauge jobs.
How about Panorama Cotton? Only 5000 copies printed.

Seriously though, I would say that these are pretty rare:
Any prototype of a game that is unreleased and has not been dumped online
World Championship Soccer 2 PAL version [Blue Box] (apparently only two have been found so far, the classics reprint and the US version are fairly common though)
Tetris (there are Chinese bootlegs with hacked copyrights, if you can't find the original Japanese version which around 10 copies were made [most likely more if you ask me])
Marble Madness (JP, different than the US/EU version due to that it is almost arcade perfect in terms of a port)
Beggar Prince, Legend of Wukong, Pier Solar aka the recent stuff especially if they are first print
The Punisher (PAL version is more rarer)
The Tec Toy Brazil stuff? Not sure, but some of these haven't been dumped as a ROM and for a couple of games, they are the only ones that run on NTSC systems (like Donald in Maui Mallard and Power Drive).
As for pirates/unlicensed games that might be rare:
Tiny Toon Adventures 3 (not Buster Busts Loose, a different game with a gorilla has a boss)
Soul Edge vs. Samurai Spirits
Any of the Gamtec stuff (excluding Squirrel King and the Mario hack of Squirrel King which is actually the most common of the Gamtec games)
Games that aren't really classed as rare, just uncommon or hard to find but there is a high demand for them, hence the high price:
Alien Soldier (EU/JP)
Mega Man: The Wily Wars (EU/JP)
Eliminate Down (JP)
MUSHA
Yu Yu Hakusho: Sunset Fighters (Brazil/JP)
Bare Knuckle III? Not sure

Revenge of Shinobi Revision 00 (the one with Batman, Spiderman and Godzilla unaltered)
There are some games that are rare as in later releases with small print runs or that the publisher was having problems but they can be found very cheaply due to virtually no demand especially if it is a sports game.
As far as rare, licensed, North American Sega Genesis, retail games that were released during the life span of the system, one game is head and shoulders rarer than any other:
Outback Joey
As far as loose cart rarity, Miracle Piano pops up only a handful of times per year, while other games listed (Splatterhouses, Punisher, Contra, Castlevania, etc.), although valuable, are very common games that can be purchased at any given time. Even if you had the money or desire to purchase Outback Joey, you'd be hard pressed to find someone willing to sell it.
Madden 93 Champion Edition was a Blockbuster Exclusive (or at least started it's life that way, we're not entirely sure) is very tough to get, the manual being the most difficult piece of the CIB.
As far as a few other notable games people should keep their eyes open for that are rarely talked about and their price does not reflect their rarity:

There are others, but that's a decent start.

I think you need to put Phantasy star 1 in the mix of the rare games for the genesis/megadrive
