According to some rarity index site from somewhere, the rarest Genesis game is Miracle Piano Teaching System. Supposedly it can go for $500 IIRC.
Not even remotely close. Complete with the entire system it is unlikely to top $250, and that's if people are bidding blindly. Cart only can be had for about $30-$40 if you wait long enough, but it only comes up a few times a year.
Outback Joey is infinitely more difficult to come by, I don't know if there are more than 10 known.
I know that it makes me more sick, basically letting Ebay rape you taking 15%. (People who constantly re-list at gauge prices are idiots in my books) If your item doesn't sell after a month or so lower the price, the eventually auction the damn thing.
There are a couple tricks to use though (don't list as a video game, list as electronic) less %fee, plus you can jack shipping up and lower the price of your item. Ebay only takes fee from price not shipping.
I paid about $4 for my Contra Hard Corps CIB. Granted, I bought the empty box and manual at a yard sale for a buck, and then got a loose cart for $3...
The PAL version of Fatal Fury 2 is rare as it only got a Australian release and never came to Europe. Its a rather limited release too, its hard enough to find one down here to begin with. In fact the only one I have personally seen is my own copy that I picked up a few years back (for a steal at only $10 too).
I've always wondered how rare the Korean version of Eliminate Down is. It was released by Samsung, and I've hardly ever seen it for sale, as it's usually the Japanese version that's being sold.

The US version of Streets of Rage 3 is bad but for collectors, it sells for around 50 bucks on average. iirc, I think it had a short print run due to the bad reviews it received due to the censorship.
Besides the obvious ones like MUSHA or the imports, etc...
Contra is actually not that rare. It's more like Gunstar Heroes. They can be pricey because of demand, but not even by so much. Actually, I got my copy of Contra for $24.99 including SnH. Gunstar Heroes was the same (both on eBay). Then I found a CIB version of GH in the wild for a buck. So there u go.
Then there are games like 3 Ninjas Kickback and Pirates! Gold. They can be pricey if you want them mint CIB. I got my copy of Pirates! Gold Mint CIB (w/ map) also for around that same price ($24.99).
Other rare items I've found you just can't find are Trouble Shooter and Master of Monsters. Now those can go for quite a lot if you want he Mint CIB.
Tiny Toon Adventures 3 is one of the rarest to find, though it's a pirate game so I dunno if that counts.
There's also supposedly a Kirby's Super Star port that I've yet to see.

It seems like everyone has their wires crossed when it comes to rare vs. hard-to-find vs. high demand. I suppose with no real agreed upon definition to go by it's to each his own.
I think Coop is on to something with the Korean version of Eliminate Down.
The aforementioned Tetris and Outback Joey are certainly tops.
I agree with MrMark about the Miracle Piano. I just picked up a complete one from eBay a couple months ago CIB in darned good condition for $160 shipped.
I have another one to toss into the mix though: "Mallet Legend"
It is the re-titled version of Whac-A-Critter that only came packed with the super-duper-uber-rare Mallet Legend controller!!! OoOoO!

The bottom 3 aren't really super rare, but will cost you around $50 at least if you want them CIB.
I think the rarity lists will always change sans games that had limited runs, prototypes, and what not.
I do remember back in 2001-02 picking up both Contra HC and Castlevania BL for $15 CIB and I saw them since go up double or more. I can also say the same for Musha as the game has since the early 2000's just go up to what I believe is a ludicrous price. But hey when demand is there games gain some type of allure and just skyrocket in price.
But I will agree that Tetris, Outback Joey, and Miracle Piano are rare and Mallet Legend I would toss in that mix as well. But Panorama Cotton with the tea cup is another too now that I also think about it, since I never have seen it since I picked one up back in the late 90's
