Ok, sorry if this question has been asked before but I was unable to find the answer. What is the difference between the regions? Is this specifically a packaging issue or will there be language and compatibility issues?
Just wanted to clear this up before trying to place an order.

The packaging is different and the languages are different. The languages are English for all three, + Japanese for JP version, + Portoguese and possibly Spanish for US version and + French and possibly German for EU version. All versions are playable on the PAL/JP Mega Drives and the American Genesis.

For the US version you definitely need Spanish since there are far more Spanish speakers than English here. You also need to advertise it! "Buy Pier Solaro for your Sega Genesiso!" I don't like languages based on masculine and feminine forms. Ends in "o" it is masculine, ends in "a" it is feminine. God damn what a shitty language! I had to take it in high school so at least I can order a meal at Chipotle as a result.
English is and always will be the best language EVAR! That's why I speak it.

I was in line there once ordering a burrito. My turn came, I was asked what I wanted and for some reason I can't explain I responded "carneeetaaaass!", rolling the R and stretching the E and S. The mexican dude behind the counter seemed like he wanted to punch me. I tried to order the rest of the fixings as normally as possibly, passing on the pico de gallo for fear of how I might pronounce it.
From the piersolar forums, the US version *has* spanish, and *might* have portuguese.
I thought they were going with Portuguese because Brazil and other South American countries were big on sega and mostly spoke Portuguese. I wouldn't include Spanish just because we have a lot of Mexican immigrants. They all need to learn the language anyway.

They probably could have supported all languages available through a single cart by putting them on the CD.
I don't think it's possible to stream that sort of data from a CD.
Are the languages using any sort of compression btw? Since all the scripts must take up a lot of room, I imagine there must be some compression to help with that.
Brazil is the only country in South America where they mainly speak Portuguese, but it is also the biggest, especially considering the MD audience. That's also the reason why it has priority over Spanish.
The texts do use compression.
