One of my top 4 favorite games ever, one of the best, cleanest and catchiest soundtracks on the system.
I chose the most peculiar but the ost is extremely varied, has 23 tracks and even the Data Select tune is awesome.
The 3 boss themes have that characteristic gnarly MD feel.
I love this game, felt like sharing a bit about the soundtrack since I just finally got the (jp version, mint, complete) after only having it borrowed as a child (eu version)

It's amazing how much better Battle 3 sounds on a real-hardware recording vs emulation. Emulated that guitar just grinds yours ears to dust, on that recording it's a lot smoother and actually nice to listen to.

I can't say what Genesis game have the best music since there are a lot of games with great soundtracks. Other than the exception some games, most of the games using the GEMS had forgettable soundtracks and never seemed to put the Genesis sound to its fullest potential.
I know it's cheating, but I just discovered this and most of the tracks sound great (PC-98 to MD conversions):

I think I might have to give Lightening Force/Thunder Force IV the nod because not only are the compositions good, the programming is great and the music greatly enhances the experience. There are other great choices too, such as the first two Streets of Rage games.
However, I want to touch on a game that I didn't see brought up (I only typed a search for the thread, so I could have easily missed it). It's a game that I got on the cheap knowing how terrible it was supposed to be. I popped the cartridge in my Model 1 , and the first thing I saw was a little attack on the SEGA logo at the beginning, which brought my hopes up a little higher. Then the title screen came up. I always like to stick around there when I first play a game. You might get some cut scenes like in Gaiares or a silly little intro that shows you some game mechanics like in The Lost Vikings, but in this case, it was just music and a screen. And the music drew me in. It made me think that people wrote this game off too early and that it had untapped potential. It made me think that there was greatness on the other side of this screen. After listening to the title track a full time through and then a full time through again to pump up myself even more, I played X-Perts.

Thunder Force IV is my absolute favorite Mega Drive soundtrack but most of Hitoshi Sakimoto's works come close to that. Not a huge fan of Verytexs soundtrack overall (Stage 1 is jamming though) but on the flipside, there's not a single bad or even okay song in Devilish at all and the programming in it is amazing.
