The entire grading industry has gotten rediculous. I collect GI Joes, so I run into a lot of Star Wars collectors. I can't tell you the amount of people that have sent in repackaged figures on fake cards that they made themselves that have been graded perfect by some services.
Also, when it comes to comics, most of the new ones that are graded gem mint are sent in by places like Wizard (who creates the price guide), then sells them based on the prices that they create. How do they get these perfect copies? By being assbuddies to Joe Quesada (Marvel's EIC) and Brian Bendis (Marvel's top writer and Quesada's best friend). They split the profits with them. (I know that sounds like a conspiracy theory, but it really does happen). They did the same thing with Image books when they first came out (except the grading aspect), they got millions of copies of the #1's and priced them extremely high so they could sell their copies at shows.
